The details of how a tick bite leads to allergic reactions to red meat and other substances are not completely understood.

The species of ticks that cause alpha-gal allergy include:

  1. The species Ixodes australiensis,found in Australia
  2. The paralysis tick (Ixodes holocyclus) found in Australia
  3. The lone star tick (Amblyomma americanum)found in theUnited States
  4. The castor bean tick (Ixodes ricinus) found in Europe
  5. An unknown species found in South Africa and other regions [6].

The tick injects saliva containing alpha-gal into the skin of its victim.

Sensitisation to alpha-gal is associated with the development of specific IgE antibodies to the carbohydrate. The component of alpha-gal that causes sensitisation is not known [7].

The symptoms of the subsequent reaction to red meat are typical of most food allergies, except the reaction is delayed by several hours and the antigen is a carbohydrate in the meat rather than a protein. More severe reactions can happen in people exposed to multiple tick bites.

Red meat is defined as every meats obtained from mammals and includes beef, pork, and lamb. The meat is red because of the high concentrations of myoglobin. Meat obtained from birds such as chicken or turkey is white meat and does not generate an allergic response due to lack of alpha-gal.

Other substances can cause the same allergic response.

  1. Intravenous fluid replacements that contain gelatine may cause reactions in individuals sensitised to alpha-gal [8].
  2. Cetuximab (an anti-cancer drug) is a chimaeric mouse and human monoclonalantibody and contains alpha-gal.
  3. Anticoagulants and bioprosthetic heart valves made from porcine intestines contain alpha-gal.
  4. Alpha-gal in gelatin, milk, whipped cream, and cheese could also elicit an allergic response [9].


The Food Safety and Inspection Service (FSIS), part of the U.S.

Department of Agriculture (USDA), is charged with ensuring the safety of meat, poultry, some fish, and processed egg products. In pursuing that goal, it regularly inspects examples of these items — sometimes responding to consumer complaints and sometimes to information from producers or distributors. The FSIS issues recalls for a variety of reasons.

The recalls go out to manufacturers, distributors, and retailers of the affected products, and the information on the recalls and other health alerts is available to the public online at the FSIS site. Consumers are asked to return recalled items to their retailers or discard them, while retailers and distributors return them to the manufacturers.

Foods are often recalled because they are found to include foreign matter (metal fragments, wood, plastic, etc.), or test positive for bacterial contamination — mostly some variety of Salmonella or E.

coli. In other cases, foods are recalled because they were not submitted for inspection — an issue particularly with frozen fish imported from Vietnam.

The FSIS recalls are classified by severity/health impact into three classes: Class 3 recalls means are of products that won’t cause adverse health consequences if consumed; Class 2 recalls encompass situations where there is a remote possibility of adverse health consequences; Class 1 is the genuine harm zone, used for cases where there is a reasonable probability that eating the product will cause serious adverse health consequences or even death.

Tick bite induced red meat allergy is an allergy to the carbohydrate galactose-alpha-1,3-galactose (also known as alpha-gal), which is present in both tick bite saliva and red meat [1]. Tick bite induced red meat allergy is also called alpha-gal allergy.

Sensitisation to alpha-gal follows a bite from certain species of tick. Subsequently, eating red meat may result in itching, urticaria, angioedema, gastrointestinal upset, and rarely, anaphylaxis [2].

The incidence of tick bite induced red meat allergy is unknown [1].

It is associated with a history of tick bites, atopy, and cat ownership [3]. Multiple tick bites increase the risk.

The allergy arises in people exposed to the ticks due to geographic location or occupation. For example, forest service employees and hunters in certain countries are at higher risk of carrying alpha-gal‐specific IgE and developing red meat allergy compared to a resident population in the same region [4].
